Part-time (up to 42hrs bi-weekly), non-benefited
Grant Funded Position.
Duties:
Movement Disorders Program at St. Joseph's/Candler is the area leader in the diagnosis and treatment of Parkinson's disease, Multiple Sclerosis, Essential Tremor and other movement disorders. The Program offers a caring, convenient and comprehensive approach to diagnosis, treatment and support services for patients with abnormal movement due to neurological disorders.
The Social Worker for the Movement Disorders Program (MDP) serves as an integral part of the Multidisciplinary Team and works collaboratively with MDP Medical Director and Outreach Coordinator to develop strategies for increasing accessibility of SJ/C services for local and regional communities. The Social Worker serves as navigator and patient advocate and is responsible for providing direct services to patients and caregivers including educational, psychosocial, and financial counseling. In addition, the position assists with providing community outreach through development of community and hospital-based educational events, awareness activities, and identifying opportunities for program funding. The SW supports the MDP by displaying effective and collaborative interpersonal skills and strong community networking while maintaining ability to work independently.
Requirements
Degree/Diploma Required/Preferred.
Bachelors required.
Masters Preferred.
Additional Education Information
Social work degree or other health related counseling or allied field
Experience
Years of Experience Required/Preferred
3-5 years required. Medical social work or community outreach.
