Summary

Duties
 
The Case Services Assistant performs certain case management functions in collaboration with Source Manager & Case Management Staff for Candler SOURCE enrollees to assist members achieve outcomes specified in Individual Care Paths to maximize optimal health and functional status.
 
Daily logging of case managers schedules while in the field.  Maintains records/logs of all case managers checking in and out times to monitor location of case manager to ensure safety.
 
Answers incoming telephone calls for GAINF and case management programs.  Operate switchboard at front desk to properly route calls to staff.  Take clear, concise messages for staff.
 
Assists Case Management staff.  Obtain medical records and develop new member files.  Follow up on member questions and concerns.
 
Develops cohesive working relationship with potential member, formal and informal support.  Establish rapport with SOURCE members, caregivers, physicians, providers, and key players to facilitate coordination of services and customer satisfaction.  Under direct supervision of manager performs and assists with screening obtain Determination of Need scores per state guidelines.  Update and maintain waiting list.
 
Complies with other administrative duties inherent in the job by assisting the manager.  Assist project manager in reviewing monthly provider reports with current data to insure reliability of provider billing recap data.  Participate in meetings monthly and notify provider of inconsistencies to address issues.  Notifying manager related to meeting finds.
 
 
Requirements
 
High School diploma preferred.
Additional business, public relations, and computer education certificates preferred.
1 to 2 years' case management, home health or related setting experience preferred.
Ability to work with indirect supervision, strong organizational skills, strong verbal and written communication, ability to self direct work activity, schedule and prioritize.
