Summary

  • Position Summary
    • To manage a caseload of elderly or disabled persons, through St. Joseph's/Candler SOURCE or the Independent Waiver Program, promoting health and functional status and continued residence in the community for all members.  Ability to work independently within assigned deadlines.  Strong organizational skills.  Knowledge of local, state, and regional health and social services.  Strong skills in verbal and written communication (including coordination with co-workers, SOURCE members, and advocates, physicians, providers, and community members).  Professional and personal commitment to quality, advocacy, productivity, creativity, and ethics.
  • Education
    • Bachelors in Sociology - Preferred
  • Experience
    • 5 - 7 Years healthcare or human services for the elderly or disabled - Preferred
  • License & Certification
    • Certified Case Management, professional license in any clinical practice a plus
  • Core Job Functions
    • Performs comprehensive assessments geared towards developing care paths. Performs quarterly reviews based on outcomes sought in care paths. Utilizes related documentation practices.
    • Develops care paths based on level of need. Clearly identifies roles for paid/professional assistance, including both direct service and education services. Adapts care path in responses to changes in an individual's circumstances or carepath variances.
    • Demonstrates the knowledge and skills necessary to identify each person's needs and provide care that contributes to achieving outcomes sought by care paths. Understands the importance of member satisfaction and excellence in customer service and how their individual actions affect departmental performance. Rapport established with member/caregiver, physician's and providers to facilitate coordination of services.
    • Communicates with PCP and attends conferences with PCP's. Makes referrals to and attends conferences with support services providers and communicates as indicated. Conference monthly for each member with supervisor.
    • Information documented in the member's chart is relevant, timely and writing in a concise and clear manner per department policy. Participates in various team functions to coordinate, plan and improve project processes and outcomes. Timely submit congruent assessment documentation by upload to GA MMIS web portal.
