Summary

  • Position Summary
    • The Ambulatory Navigator will provide support to the Ambulatory Care Coordinators and the physician offices within MGM and the Advocate Health Network. The Ambulatory Navigator works in collaboration with the Clinical Quality Manager. The Navigator will have a presence in the physician offices to ensure annual wellness completion, post hospitalization discharge visits, monitor quality measure and tracking of outcomes for quality reporting.   The Navigator will have an understanding of managed care programs, medical terminology, and medical coding.  The Ambulatory Navigator reports to the Clinical Quality Manager.

    • 2 - 3 Years Healthcare - Required 
    • 2 - 3 Years Excel - Required

  • Core Job Functions
    • Serves as a patient educator-advocate and patient empowerment facilitator. Educates and encourages patients to self-management to identify disease exacerbations with goal toward early intervention and wellness.
    • Monitor scheduling and completion of Annual Wellness visits. Monitors post hospitalization PCP visits following hospitalization or emergency department visit. Assist with electronic recording of data for collection, storage, analysis, retrieval and reporting.
    • Spend up to three days per week with physicians in their offices. Maintain strong communications with other members of the care coordination team and the practices assigned. Train and provide above services to physicians and practice managers new to the ACO or MGM.
