Position Summary
-
Certified Professional Coder (CPC) or Certified Coding Specialist (CCS) to provide quality review and analysis of a wide range of patient medical records and ensure accuracy of coding in accordance with accepted medical and legal standards. Responsible for reviewing physician chosen CPT, HCPCS and ICD-10-CM codes to the physicians' documentation to substantiate the level of coding, and physician specialty outpatient encounters; including, but not limited to: E/M clinic encounters, infusion services, procedures within specialty practices and the related infusion therapy suites. The Outpatient Medical Coder will abstract other data elements as required and work within the department and with the practices to obtain any necessary information to accurately and correctly code and bill claims to payers. Other tasks may include account creation and charge entry as needed.

Core Job Functions

Assigns diagnosis and procedure codes to complex outpatient encounters according to regulatory guidelines. Physicians are queried for missing documentation to assure accurate coding, and an account is not finalized until a response is received. Other key data elements are abstracted to provide information for regulatory reporting. Uses encoder to invoke national correct coding and outpatient code edits and applies modifiers and edits appropriately.

Maintains certification and engages in continuing education activities. Stays up-to-date on coding changes, Coding Clinic advice, and payor regulations including national and local policies. Shares knowledge with the rest of the team.
- Reviews records for additional information for accuracy and to ensure all data elements are present as required for claim submission.
- Assist in charge entry initiatives/special projects and assist in coding and charge entry coverage as needed to keep skills current. Provide assistance with highly complex coding as workload permits.
