Summary

• Position Summary
  • Primary care practitioner in a free clinic serving the uninsured and low income populations. Duties include patient assessment, diagnosis, treatment, completing orders & follow up, patient education, prescriptions. Initiate referrals to, and with, specialty care physicians. Member of an Interprofessional team sharing knowledge and decision making.
• Education
  • Masters in Nursing - Required
• Experience
  • 1-2 Years experience in a clinic or private practice - Preferred
• License & Certification
  • Professional License with State of Practice - Required
  • Advanced Practice - NP - Required
• Core Job Functions
  • Order labs, imaging, diagnostic, medicines appropriately with focus on cost and first step of action, rather than most expensive first. Sends in prescriptions and refills in a timely manner.
  • Review results of orders in a timely manner and uses results to plan care. Instructs patients on the results, plan of care and outcome expectations.
  • Investigates thoroughly before requesting specialists. Able to provide referral with all tests, action taken to diagnose and questions regarding irregular outcomes.  Willing to co-manage with specialists.
  • Incorporates SDoH in to the diagnosis and treatment. Documents major barriers that could contribute to the health problems.  Refers to Social Worker or others for Community Resources.
  • Documents appropriately and timely. Researches and consults as needed.  Includes the patient in decision making and holds them accountable for agreed outcomes.
