Summary

  • Position Summary
    • Under the general direction of the Program Director, provides advanced pharmaceutical care according to standards established by the Department of Clinical Pharmacy to all patients. Assists in the development and coordination of clinical pharmacy services and educational programs for the Department. Responsible for providing tools and educational opportunities to enhance services provided by all staff members. Participates on intra- and interdepartmental committees as assigned. Serves as preceptor to pharmacy students, and acts as Chief Resident for PGY-1 Pharmacy Residents. Participates in the development of institution-wide drug use policy. Responsible for ensuring safe and efficient drug distribution and control system. Participates in supervision of technical staff. Participates in clinical research with medical, dental and other Health System staff. Responsible for completing a research project suitable for publication.

    • Doctorate, Pharmacy - Required
  • Experience
    • 1-2 Years - Required
    • Completion of clinical rotations necessary for graduation from an accredited college of pharmacy
    • Completion of all clinical rotations necessary for graduation from an accredited PGY-1 Pharmacy Practice Residency
  • License & Certification
    • Professional License with State of Practice - Required
  • Core Job Functions
    • Provides consultation/intervention on non-formulary drug requests. Patients are counseled regarding dosage therapy, side effects and interactions related to discharge medications. Provides in-service education to hospital personnel regarding new medications as needed
    • Assess pharmacotherapy and individualize therapy to specific patient needs and communicate therapeutic recommendations to the Medical Staff. Establish and clarify goals and objectives of therapy in collaboration with the physician for each problem being treated. Provide patient education and counseling and participate in discharge planning for the purpose of ensuring continuity of drug therapy.
    • Develop and implement clinical pharmacy training programs for staff pharmacists, Residents and Nursing as needed. Provides supervision and instruction of PharmD students and Residents on clinical rotations as assigned. Attend and participate in Pharmacy Leadership meetings.
    • Medications are dispensed according to established procedure and State/Federal regulations. Performs clinical services to optimize drug therapy. Clinical assessment decisions are made to include appropriate cost-effective therapy to ensure excellent patient outcomes.
    • Inpatient medication profile is monitored for drug interactions, food/drug interactions, duplicate therapy, and other potential medication problems in drug therapy. Assures that central pharmacy, nursing stations and ancillary areas use appropriate methods for medication storage.
    • Provides supervision and instruction of PhamrD students on clinical rotations as assigned. Precepts PharmD Residents as assigned providing supervision and mentorship. Participates in the planning and evaluations of PharmD students and Residents as needed.
    • Solutions are prepared, labeled and available according to policy guidelines and physician orders. Assures that aseptic technique is followed during preparation.
