Summary

  • Position Summary
    • This position will interface with the breast health team and assist with the coordination of all aspects of diagnostic imaging, clinical evaluation, support services and education for women receiving services at the breast center. This position provides clinical functions as required in support of breast center services and participates in maintaining a safe, cost-effective environment that supports the mission, vision, and values of St. Joseph's / Candler.

    • Associates in Nursing - Required
  • Experience
    • 3-5 Years Breast Center or Breast Imaging - Preferred
    • 2-3 Years Breast Patient Navigation - Preferred
  • License & Certification
    • Professional License with State of Practice - Required
    • National Certification - Preferred
  • Core Job Functions
    • Demonstrates knowledge and skills to provide care appropriate to any age-related needs of the patient being served. Coordinates patient care needs with team members and other appropriate departments. Coordinates/oversees patient placement within the system and monitors house availability and assesses patient needs for house placement.
    • Reviews all new and follow-up cases daily. Makes required follow-up appointments for patients and communicates appointment status to patient and referring physicians.
    • Accurately enters procedural documentation and patient charges per department guidelines. Special needs or concerns of the patient are identified and communicated to the radiologist/referring physician to ensure proper action. Follow-up appointments and outcomes are documented and appropriate actions taken to ensure all outstanding follow-ups are completed or reasons for deferral are documented.
    • Approaches and negotiates navigation agreements with new physicians and updates existing agreements as requested. Maintains documentation for outliers to agreement parameters.
    • Maintains up to date knowledge of policies and procedures of Hospital as well as Standards of Practice for specialty area. Exercise independent judgment in the assessment of patient's needs, and identifies clinical priorities throughout the Hospital.  Responds appropriately to resolving issues that impact patient care. 
    • Knowledgeable of Joint Commission, OSHA, FDA & ACR regulatory guidelines concerning the care of the breast imaging patient. Participates in quality improvement processes and creation/revision of policies and guidelines to support best practices.
