Summary

  • Position Summary
    • The Manager for Care Coordination is responsible for day to day operations of the acute care case management practice. This includes assessing, planning, implementing and evaluating the delivery of case management services and providing for the development, evaluation and education of the case management personnel. The Manager for Case Management collaborates with other departments, community agencies and physicians to impact positive patient outcomes as a result of system leadership and case management interventions.
  • Education
    • Bachelors of Nursing or Social Work - Required
    • Masters of Nursing  or Social Work - Preferred
  • Experience
    • 3-5 Years Hospital based experience - Required
    • 5-7 Years Case Management, Utilization Review, Discharge Planning - Preferred
    • 2-3 Years Insurance requirements - Preferred
  • License & Certification
    • Registered Nurse License with State of Practice - Required for nurse candidates
    • Licensed Clinical Social Worker (LCSW) - Required for social worker candidates
    • Case Management Certification or similar category - Preferred
  • Core Job Functions
    • Assists Care Coordinators with identification of patients not at acute level of care. Assists CCC staff with setting daily priorities to maximize resource utilization. Identifies barriers to LOS, discharge planning, and patient throughput.
    • Obtains patient care information through daily unit reports, patient rounds, focus review rounds, discharge rounds. Responds to requests of case escalation to assist in complex patient discharge planning. Track and analyze overall quality patient care, identify and respond to problems.
    • Maintains knowledge of staff educational needs and activities related to the unit. Oversees and plans for unit staff educational needs including new hires orientation. Coaching and counseling of with staff for development needs, career planning, and to ensure quality patient care. Coaching staff in the moment as staff delivers patient care.
    • Provide leadership to staff in goal setting, problem solving, resource management, and outcome achievement. Demonstrate fiscal accountability for department resources and the ability to achieve outcomes within allocated resources. Demonstrates a clear understanding of regulations applicable to patient care and/or department functions.
    • Defines performance objectives and metrics for the department and assesses the level of competence of staff. Prepare and review performance evaluations, as scheduled with case management staff. Develops performance goals/objectives with each staff member to promote maximum productivity, proficiency and professional growth.
