Summary

  • Position Summary
    • APP will evaluate patients, counsel patients, family, and caretakers in both ambulatory clinic and post surgical / CCU setting. APP will serve as extension of surgeon and assist with post-operative oversite of patients, ongoing clinical assessment, pre-operative evaluation, collaboration/communication with surgeon and CTS team, and patient education.

    • Facilitate referrals of patients to other medical providers, testing, medical facilities, or other health/social service agencies when appropriate and as delegated by supervising physician. Play active role in continuous patient assessments, discharge planning and post follow-up coordination.

    • APP will work in a highly team-oriented setting, and participate in clinic schedule coverage, including patient office visits, new patient work ups, and post operative care and coverage in extenuating circumstances. APP will also participate in special projects, and assist in clerical work surrounding patient care coordination, communication, triage, handoffs, and procedural authorizations.

    • Masters of Nursing - Required
  • Experience
    • 1 Year Clinical Practice - Preferred
  • License & Certification
    • Advance Practice License GA (PA candidates will be considered in lieu of NP) - Required
    • Basic Life Support (BLS) - Required
  • Core Job Functions
    • Provide guidance on available services and self management. Assist patients in navigating health education and supportive services within and outside of clinic, including the facilitation of referrals. Follow-up on care and ensure that patients are receiving appropriate care.
    • Administers medication safely. Provides quality care to all patients. Takes initiative and uses sound judgment.
    • Follow policies and procedures for routine office procedures, test, administration of medications and assisting providers up to and including IV therapy. Recognizes and anticipates urgent and emergent patient situations and responds appropriately.
    • Identifies patient education needs and may assist in patient teaching as to appropriate health maintenance and management of illness. Communicates effectively with patients, families, and other members of the health care team.
    • Informs patients on physician instructions and any preparations. Informs patients about procedures prior to performing procedures. Explains consent forms to patients and has patients sign consent forms.
    • Vital signs are taken on each patient, on each visit and input into eClinical Works. Verifies and updates patients clinical record on each visit.
    • Refers patients to correct laboratory, radiology center, hospital or specialist. Ensures precertification is obtained when needed prior to services being rendered. Follows referral process based upon insurance company's policies and procedures.
