Summary

  • Position Summary
    • The Patient Placement Coordinator is responsible for taking calls from physician's or designee to schedule an admission, obtain demographic information and communication to the physician offices of the rooms assigned for the incoming patients. The Patient Placement Coordinator also has the responsibility to contact any referring facilities with the notification of rooms assigned in order to expedite the transfer of patients to our facility. The coordinator initiates the patient placement process by identifying an appropriate bed and contacting the charge nurse of the unit to coordinate the room assignment for all patients entering the system. Additional functions include transferring existing patients to different areas, updating patient information in the computer, and communication to appropriate personnel regarding overall census and the availability of beds and prioritizing the need for rooms to be cleaned for Environmental Control. The Patient Placement Coordinator will provide excellent internal and external customer service and maintain a working knowledge of all hospital and departmental policies and procedures and professional standards

    • 1-2 Years Hospital environment - Preferred

  • Core Job Functions
    • Calls for admissions are answered timely. Maintains accurate records of information obtained and recorded for patient admissions. Timely input of information into tracking system. Orders are printed to correct location and receipt verified for new admissions.
    • Follows the established policies to make patient assignments. Coordinates the movement of patients to assist with bed availability. Prioritizes the cleaning needs of patient rooms.
    • Accurate and timely computer functions carried out for new admissions and transfers. Must demonstrate knowledge of patient accommodation codes as relates to computer entry.
    • Calls are answered and processed timely. Maintains a log of all calls received. Keep current on-call list posted daily. Contact on-call physician timely with appropriate information provided.
