CCSP Care Coordinator

• Position Summary
  • Manages a caseload of elderly and/or disable persons, through St. Joseph/Candler CCSP, promoting optimal health and functional status and continued residence in the community for all clients.

  • Bachelors in Liberal Arts - Required
• Experience
  • 1 Year Social services or human services - Required
  • Nursing or experience in human services or psychology.

• Core Job Functions
  • Information documented in member's chart is relevant and timely, in a concise and clear manner per department policy. Case conference monthly with supervisor. Provides timely documentation to providers to broker services and provide information regarding changes of client status to prevent interruption of service.
  • Assists in development of care plans based on level of need. Evaluate client's medical and health status, functional ability, social, emotional, and environmental factors related to illness, support system, home environment, formal and informal support in order to justify the need for CCSP services and meet need. Participates in multidisciplinary team meeting.
  • Demonstrates knowledge and skills necessary to identify each person's needs and provide care that contributes to outcomes sought by care plans. Understand the importance of client satisfaction and excellence in customer service and how their individual actions affect departmental performance. Rapport established with client/caregiver, physicians and providers to facilitate coordination of service.
  • Performs monthly contacts and quarterly reviews based on outcomes sought in case plans. Adapts care path in response to changes in an individual's circumstances or care plan variances. Utilizes related documentation process. Annually prepares client to submit financial documentation for cost share amount.
