Summary

  • Position Summary
    • The Quality Improvement Coordinator will have the primary responsibility of coordinating key quality improvement (QI) activities and projects throughout Medical Group Management (MGM) practices. This position will assist in data and material management for various quality based insurance programs to ensure compliance across MGM and will assist as needed with coordination of care while supporting patients and families in such areas as; self-management, self-efficacy and behavior change. Position requires individual to exhibit strong communication and analytical skills along with proficiency in Microsoft Excel. Individual must practice concepts of population management and address needs of patient and families. The position will report directly to the Medical Home Manager.

    • 2-3 Years Healthcare - Required
    • 2-3 Years Excel - Required

  • Core Job Functions
    • Analyzes and constructs project level qualitative and quantitative data for reporting purposes to MGM and outside entities.
    • Arranges and facilitates monthly collaborative calls as required by project deliverables.
    • Contributes to the development of expert knowledge on quality improvement frameworks i.e.(CMS, NCQA and commercial entities).
    • Provides technical assistance to MGM offices to meet reporting requirements within defined timeframes for assigned projects.
